1. Insert not allowed SIM card.
2. Phone will tell you to insert an allowed SIM card and that the phone will switch back to CDMA mode.
3. Click OK.
4. Click green button to get to the main screen of the phone.
5. Click on settings.
6. Click on all settings.
7. In the personal tab click on security.
8. The phone will ask you to enter the unlock code. Enter it. Done.

Thanks for your post above. I have HTC incredible 2 for dual CDMA-GSM/LTE, the phone is unlocked for GSM SIM Card only, CDMA SIM Card not working.

The SIM Card received from Verizon it is meant to be used only when abroad fro Roaming purposes. Actually it is a Vodafone GSM Sim Card.
In order to use it with another CDMA provider you need to contact the new carrier and provide them some information about your handset. They will activate it without the need of a SIM card.
trying to unlock my Rezound and none of the setting used your unlock description are the same in the rezound?? and it never prompts me to put in the unlock code.. and I've allready purchased the unlock code. im confused??
Same here Rezound is not asking for unlock code.

Got a Rezound in Denmark (Europe).
The unlock procedure you provide for the Rezound, is not matching what is actual on that phone (running default Verizon software config currently)
Here's what I have tried, after purchasing the unlock code from you:

You won't be prompted for an unlock code until you click on disable SIM card lock. Then you enter the unlock code, your phone will reboot and you're all set.
